school codes and district names

finally getting around to doing my taxes and I noticed that the IT-150 asks for school district name and code number. I can't find this information anywhere so does anyone know what the code and name for either 11215 or the area around PS 321? thanks so much. if no one has it i'll call the school tomorrow....

You need to use the instructions. It's in there, trust me (but the poster above is also correct).

Taxact.com figured all this stuff out for me. I simply plugged in my address and numbers and taxact.com did all the rest.
