Geriatric Assessment: A Road Map for Independent Living

The Senior Umbrella Network of Brooklyn (SUN-B) is hosting a seminar on geriatric assessment as a roadmap for independent living among the elderly population. The featured speaker is Alexis Carter, Director of Senior Source at Selfhelp Community Services, who will discuss the various clinical tools used and observations made during the assessment process, and how this information is key to a successful transition from one setting of care to another. She will also discuss what a care plan can offer, citing real world examples of how effective care management is keeping our fellow New Yorkers safely in the community for as long as possible.
